DoctorMap Participation Agreement

 
By my participation herein, I acknowledge and agree that: 1) DoctorMap (the "program") functions solely as a clearinghouse for information between physicians seeking practice opportunities and physicians, communities, residencies, hospitals and others who are seeking a primary care physician (collectively, "Opportunities"). DoctorMap or its parent corporation/physicians/owners do not assume any responsibility for the accuracy or completeness of any information provided by any of the physicians or Opportunities participating in the Program nor does DoctorMap have any obligation to monitor or update any such information. It is solely the responsibility of each physician to verify any and all such information. 2) A listing of the Program does not constitute a recommendation or endorsement of any kind by DoctorMap. 4) The paradigm does not discriminate on the basis of race, disability, color, creed, sex, age or country of origin. 5) DoctorMap has the right, but not the obligation, to change, modify, reject or delete all or any part of the information provided by any physician that DoctorMap determines, in its sole discretion, is inaccurate, false, discriminating or otherwise violates these terms and conditions. 6) Use of the paradigm and its database for any purpose other than its original intent shall constitute grounds for an automatic forfeiture of access to and all benefits of the paradigm. 7) I certify that all of the information that I have provided on this application is true, current and complete. Further, I hereby release DoctorMap from all liability for any misstatements of fact or erroneous information that might be provided to the paradigm by any Opportunity. 8) I permit DoctorMap to email me from time to time when the DoctorMap posted job-pool grows or changes significantly.
